London Ambulance Service CAD Failure, 1992

In October 1992, London Ambulance Service (LAS) Computer Aided Despatch was made operational. One day later, the system was shut down due to some errors in the Automatic Vehicle Locating System (AVLS). The system could not keep track of the location and status of units which led the system to slowing down and failure.
